Before visiting and/or reserving products at an Ethos dispensary, you must become a certified patient or caregiver under Ohio’s Medical Marijuana Control Program. The steps for becoming a patient/caregiver in Ohio are discussed below. To become a certified patient or caregiver, you or the patient for which you are providing care must have one of the twenty-five qualifying serious medical conditions. - Inflammatory Bowel Disease
- Multiple Sclerosis
- Pain (chronic and/or intractable)
- Parkinson’s Disease
- Positive Status for HIV
- PTSD
- Sickle Cell Anemia
- Spasticity
- Spinal Cord Injury and/or Disease
- Terminal Illness
- Tourette’s Syndrome
- Traumatic Brain Injury
- Ulcerative Colitis
Regulatory Requirements
Obtaining a Ohio medical marijuana ID card requires the following steps:
VISIT A CERTIFIED PHYSICIAN
- Visit a certified physician who can confirm you have one of the qualifying conditions for Medical Marijuana. After confirming your condition, the physician will create your profile in the Patient & Caregiver Registry. Please note: You will need to present a valid Ohio Driver’s License, a valid Ohio identification card distributed by the Ohio Bureau of Motor Vehicles (BMV), or a valid United States Passport.
COMPLETE YOUR REGISTRATION
- After your certified physician creates your profile in the Patient & Caregiver Registry, you will receive an email to confirm and complete your registration. This email will go to the email you provided your certified physician when you were registered.
MAKE PAYMENT
- Once logged into the Patient & Caregiver Registry and completed your application you will need to pay the registration fee. You can log into the Patient Registry here. The fee is $50 for patients and $25 for caregivers. Note: if you are registering for a Medical Marijuana card with Indigent or Veteran status, you may qualify for fee reductions. After you’ve successfully paid, you will have the option to download and print a copy of your card or wait for the physical card to come in the mail. The physical card will be mailed to the address you provided when you were added into the Patient & Caregiver Registry.
Discounts

20% for Veterans, Supplemental Security Income (SSI), Social Security Disability Insurance (SSDI), and The Supplemental Nutrition Assistance Program (SNAP).
