  • Visit a certified physician who can confirm you have one of the qualifying conditions for Medical Marijuana. After confirming your condition, the physician will create your profile in the Patient & Caregiver Registry. Please note: You will need to present a valid Ohio Driver’s License, a valid Ohio identification card distributed by the Ohio Bureau of Motor Vehicles (BMV), or a valid United States Passport.
  • After your certified physician creates your profile in the Patient & Caregiver Registry, you will receive an email to confirm and complete your registration. This email will go to the email you provided your certified physician when you were registered.
  • Once logged into the Patient & Caregiver Registry and completed your application you will need to pay the registration fee. You can log into the Patient Registry here. The fee is $50 for patients and $25 for caregivers. Note: if you are registering for a Medical Marijuana card with Indigent or Veteran status, you may qualify for fee reductions. For more information of fee reductions, click here. After you’ve successfully paid, you will have the option to download and print a copy of your card or wait for the physical card to come in the mail.
